How to Make a Referral

In order to expedite the referral process, we ask our referral partners to please include the following:

  • Referral source contact information
  • Insurance/Funding information
  • Medical issues
  • Recent psychiatric evaluation
  • Psychological testing
  • IEP documentation
  • Treatment plan from current placement
  • Discharge summaries from prior treatment
  • Other relevant information to determine eligibility
